To schedule an appointment contact Elizabeth Neumann, RD. LD by phone 817-995-4677 or email . We can meet in your home, a local business or 4895 E. I-20 Service Road North Willow Park, Texas 76087 Choose the place you will feel most comfortable Office hours are Tuesday, Thursday and Saturdays by appointment only.  Prior to your first appointment, there are several forms to complete that will help me with your initial nutrition assessment. Once we have scheduled an appointment, I will email you a Nutrition Packet/Questionnaire and a Physician Referral Form.  Please complete all forms and have them available at your first appointment.  Also, any lab work, medication list and supplement list would be helpful. At your first appointment we will review a diet history and determine dietary changes and strategies to achieve a healthy diet.  Follow-up appointment may be needed to determine progress and provide an opportunity for continued learning.  If you need to cancel or reschedule an appointment please let me know ASAP.  Schedule an appointment now so that you can change your life with Healthy Nutrition.

Medical Nutrition Therapy (MNT) includes:

Nutrition Assessment Nutrition Diagnosis Nutrition Interventions Monitoring and Evaluation MNT is provided by Registered Dietitian Nutritionist (RDL) serving North Texas.  According to the Academy of Nutrition and Dietetics:” A registered dietitian is an expert in food and nutrition and is professionally qualified to help individuals meet their nutritional goals”.   The Academy and Medicare require MNT to be evidence-based and use Nutrition Practice guidelines.  RDLs use proven strategies during nutrition counseling for maximize effectiveness. DSME is an interactive, collaborative process that can equip adults with basic knowledge to manage their Type 1 and Type 2 diabetes while focusing on their self-identified problems and goals.  This 10 Class program emphasizes problem solving and decision making as they relate to core diabetes self- care skills such as the list above.
